Bridges to Health (B2H)

Bridges to Health is a Medicaid Waiver program that provides services to children who qualify under one of three waivers: Seriously Mentally Disturbed (SED), Developmentally Disabled (DD), or Medically Fragile (Med F.

B2H provides services to children who are in foster care in settings of up to 12 children. Once enrolled in B2H, the child may continue to receive services while in care or if discharged home or adopted until the age of 21 if services continue to be necessary.

The goal of B2H is to help children function successfully in the community and to prevent placement in higher levels of care by offering services that meet the child's unique needs.
